We assess and apply a quantifiable value to each problem in order to allocate the right resources for your investigation. You will always receive cost-effective services and tangible solutions that deliver an optimum return on investment.

Gather and Manage Data

Gather, secure and manage all the supporting data connected to your problem or incident. This is the essential first step to ensure a successful RCA.

Create the Problem Statement

Identify the focal point of your problem. Your team will answer a series of questions to identify where and when the incident occurred.

Analyze Cause and Effect

Every event has multiple causes. You’ll explore transitory and non-transitory causes, omissions, key points of change.

Generate Solutions

Your team will brainstorm ways to control, alter and prevent causes and look for both preventive and corrective actions.

Produce the Final Report

The final document will include an executive summary, problem statement, cause-and-effect chart, solutions matrix,team information and key completion dates.
